Eloralintide vs Cagrilintide and Pramlintide: Amylin Receptor Selectivity

What makes eloralintide different from cagrilintide and pramlintide? Eloralintide was designed to favour the amylin 1 receptor over the calcitonin receptor. Researchers hoped that selectivity would keep appetite effects while reducing nausea-type signals. The early evidence is promising but mostly comes from cell, animal and company-sponsored studies.

This original FirstLabs editorial review was updated November 3, 2026. It is educational information, not medical advice, a dosing guide or a recommendation for personal use. FirstLabs catalogue materials are for laboratory research only, not human or veterinary use.

Amylin and calcitonin receptors explained

Amylin is a hormone released with insulin. Its receptors are built from the calcitonin receptor (CTR) paired with receptor activity-modifying proteins, creating amylin receptor subtypes such as AMY1R and AMY3R. A compound can activate these subtypes and the calcitonin receptor to different degrees.

Pramlintide was the first therapeutic amylin analogue and is used with insulin in some countries. Cagrilintide is a long-acting, non-selective amylin and calcitonin receptor agonist. Eloralintide is described as an AMY1R-selective agonist.

What the laboratory data showed

In cell assays reported by Briere and colleagues, eloralintide activated human AMY1R about 12-fold more potently than CTR and about 11-fold more potently than AMY3R. In rats, AMY1R and AMY3R were both activated more potently than CTR, so selectivity differed between species.

In lean rats, eloralintide caused significantly less conditioned taste avoidance than cagrilintide. Taste avoidance is an animal model sometimes used as a signal for nausea-like effects. In diet-induced obese rats, it reduced food intake and body weight mainly through fat-mass loss.

Does selectivity mean fewer side effects in people?

Not necessarily. In the 48-week phase 2 trial, nausea still affected 11% to 64% of participants across study groups. A 2026 review of long-acting amylin analogues noted that nausea and other gastrointestinal effects are common when these drugs are started or increased, and often ease later.

No published head-to-head human trial compares eloralintide with cagrilintide. Lower taste avoidance in rats is not evidence of better tolerability in people.

The wider amylin research pipeline

The same review lists eloralintide alongside petrelintide, MET-233 and AZD6234 as long-acting amylin analogues showing efficacy in trials. Cagrilintide has been combined with semaglutide, and amycretin combines amylin and GLP-1 activity in a single molecule. These are separate investigational programmes, not variants of one product.

Eloralintide selectivity: frequently asked questions

Is eloralintide the same as cagrilintide?

No. Both act on amylin receptors, but cagrilintide is non-selective and eloralintide favours AMY1R.

Does eloralintide cause nausea?

Yes, in human trials nausea was the most common side effect, at rates that varied by study group.

Has eloralintide been tested against cagrilintide in people?

No published head-to-head human trial was identified for this review.

Laboratory documentation is a separate question

Evidence about eloralintide in a clinical study does not establish that a FirstLabs vial is equivalent to the studied formulation. A certificate of analysis describes the submitted sample, not medical suitability or regulatory authorization. HPLC purity does not by itself establish quantity, sterility, endotoxin control or a match to current stock.

For laboratory records, verify exact identity and labelled size, analytical methods, measured quantity, sample identifiers and any available lot-to-stock documentation. A laboratory task ID is not automatically a manufacturing lot. Keep missing reports and adverse results visible rather than borrowing evidence from another size.
